I always figure you should never get under a car without a plan B. If the wheels are off, slip them in an appropriate spot in case a stand tips or fails. If wheels are on, chuck a piece of wood under each tyre to give you room to survive if the car drops.

I know what you mean - had the Kluger on stands yesterday. Even though they are rated way higher than they need to be it still feels sketchy.

hoist locking pins fail more than floor jack stands.

of course there's some defective products in the field, but there's heaps of these products that aren't on the list. Also, surely we'd see a lot more news articles of either near misses or people being crushed under their cars, but basically everybody who works on their own car uses jack stands. From my experience, there's more articles of cars falling off hoists than cars falling off jack stands.
