Not suprised that it broke in that specific place.

Stepping up and down in shaft diameters will lead to concentrations of load at transitions.

A gentle radius/ramp, rather than a chamfer with an intersection "point", would have been a better option.

Which is why the intermediate shaft broke, but the input shaft remained intact with a twist.
