Well the flapper arm itself the actuator was welded on the inside of the flapper arm instead of the outside which caused it to be crooked and wear the diaphram out over time,

still not sure how that's even possible

Obviously the flapper post broke off and some clown welded it back on wrong

Prolly welded it shut .......
