Possibly the rod was fractured or a bearing got a tiny bit of grit in it, or something that is fairly freakish....Then, while under engine breaking, there was just the right pressure, at the right frequency to make it give up... Remember, you hit the resonant frequency of a crack (yes, you can do this, the area along the crack itself has resonance) and that's it... gone.
